I'm coming in as an existing patient with a new problem 

If you are a returning patient with a new condition, depending on when you were seen last, there are certain forms you need to complete.  Please arrive at least 20 minutes prior to your appointment in order to complete these forms on time.  Also remember to bring with you:

• Your insurance card
• The referral, if it is needed
• A government approved picture ID
• A co-payment, if it is needed
