To request a copy of your medical records, please call us at (609)896-0444 and select extension 2304 during regular business hours. Please complete our medical release form, listed below. The medical release form can then be hand-carried, mailed, or faxed to our main office.

Your medical record request will be forwarded to Records Reproduction Services (RRS) for proper administration.  Safeguarding your medical information is of prime importance to MBO.  Your patience is appreciated as we comply with government HIPAA regulations.
